How Bariatric Surgery Reverses Type 2 Diabetes

Beyond weight loss, bariatric surgery changes the body's own hormone signalling in ways that can put type 2 diabetes into remission, a mechanism Dr. Rakesh Durkhure walks through using outcome data from his own practice.
The Hormones Driving Hunger and Insulin Resistance
The stomach's own hormone, ghrelin, signals the brain to eat more, and is one of the main drivers of overeating in obesity. Gastric bypass changes this hormonal environment directly: the procedure increases release of GLP-1, PYY and GIP, hormones that prompt the pancreas to produce more insulin and help bring blood sugar down, rather than only restricting food intake from the outside.
Bypass Outperforms Sleeve on Insulin Sensitivity
Gastric bypass increases insulin sensitivity 2.7 times, compared with only 1.2 times after sleeve gastrectomy. Diabetes medication can lower a patient's blood sugar reading, but according to Dr. Durkhure it does not improve the underlying sugar tolerance the way surgery does. In his data, gastric bypass produced full diabetes remission, meaning the diabetes goes away entirely, in 42 percent of cases, along with a substantial reduction in HbA1c.
Surgery Versus Ozempic and Mounjaro
Newer medications such as Ozempic and Mounjaro act on some of the same hormone pathways, reducing appetite and slowing gastric emptying while increasing insulin secretion. In India they cost around 500 dollars a month and must be taken every day, indefinitely, since stopping the medication reverses the weight loss. Surgery's cost is typically recovered within about a year of what that ongoing medication would otherwise cost, after which the only ongoing requirement is vitamins and good nutrition.

What is the risk of internal hernia following surgery, and what about the risk of leakage?
Dr. Rakesh Durkhure
I can say with complete confidence that I have never seen an internal hernia, because every possible site where one could form is repaired during surgery. Leakage is the most feared complication, and we haven't seen that either, because we run a puncture test during surgery to check for any leak before closing. I also record the video of every surgery and give it to the patient at discharge, so they can show it to their own doctor back home.

How does the cost of surgery compare with the lifetime cost of medical management, in terms of return on investment?▼
Ozempic and Mounjaro cost about $500 a month in India and have to be taken indefinitely. The cost of surgery is recovered within about a year of what that ongoing medication would have cost. After that, the only requirement is vitamins and good nutritious food.
Beyond bariatric surgery, what other conditions can African doctors refer to your service?▼
The scope goes well beyond bariatric surgery. It includes hiatus hernia, where the stomach moves from the abdomen into the chest, gallbladder stones, particularly complicated cases where a stone has slipped into the bile duct, and problems of the colon and small intestine. It also covers complex abdominal hernias, including inguinal, umbilical, and recurrent hernias that have come back after a local repair, as well as rectal prolapse, complex piles and haemorrhoids, and fistulas near the back passage. All of these can be managed and the patient sent back with a satisfactory outcome.
What are the early signs of obesity?▼
A waistband that keeps needing loosening is the earliest sign. A partner reporting loud snoring at night is another. And marked breathlessness after something as minor as running 100 metres is a third.
How affordable are these surgeries?▼
Door to door, from hospital admission to discharge, the cost runs about $5,500 to $6,000. It varies by roughly $500 to $1,000 depending on the patient's body weight, diabetes management and breathing problems.
Can bariatric surgery put type 2 diabetes into remission?▼
Yes. In Dr. Durkhure's data, gastric bypass produced full diabetes remission in 42 percent of cases, alongside a 2.7 times increase in insulin sensitivity.
