The Three Pharmacological Pathways in Pulmonary Hypertension Treatment

Medical treatment for pulmonary hypertension targets three distinct pharmacological pathways, and understanding these pathways explains why combination therapy, rather than a single drug, is the standard of care.

The first pathway is PDE5 (phosphodiesterase type 5) inhibition, which promotes vascular relaxation via the cGMP pathway. Tadalafil is the preferred starting agent in this class: 2.5 mg once daily for patients under 60 kg, and 5 mg once daily for patients 60 kg or above. Patients need to be counselled on the risk of hypotensive, syncopal episodes, and in younger male patients specifically, on the risk of prolonged erections - a side effect that should be addressed directly, with psychological support offered if needed. Tolerance to this side effect tends to develop as the dose is escalated.

The second pathway is soluble guanylate cyclase stimulation, which addresses right ventricular back-pressure and promotes vasodilation. Riociguat (marketed as Adempas) is the agent used in this class.

The third pathway is endothelin receptor blockade, which retards smooth muscle cell hypertrophy and proliferation at the vascular level, reducing both pulmonary vascular resistance and pulmonary arterial wedge pressure. Macitentan, taken at 10 mg once daily, is the preferred agent in this class, because it has been shown to reduce composite endpoints and clinical worsening events at one, three and five years. Bosentan and ambrisentan are also available in this class.

Alongside these three pathways, prostacyclin derivatives play an important role. Epoprostenol, given intravenously, dramatically improves exercise capacity. Iloprost, given by inhalation multiple times a day, is effective but commonly causes severe pruritus, dry mouth, and allergic reactions, so dose titration is essential. Selexipag, an oral prostacyclin receptor agonist, is typically dosed at 200 micrograms twice daily for most adults, or 100 micrograms twice daily for patients under 50 kg; it can cause a swollen tongue and severe pruritus in some patients. Treprostinil is another agent in this class.

The right ventricle is a volume-handling ventricle, not designed for pressure overload — you can load it with considerable volume and it won't fail, because volume is its domain. But impose a pressure overload, as in pulmonary hypertension, and it hypertrophies to compensate before eventually failing. The left ventricle is a pressure-handling ventricle, built for exactly that load. That's why pulmonary hypertension causes right heart failure long before it affects the left.

What is the mechanism of pulmonary hypertension in HIV?

Two mechanisms operate together. Viral proteins cause stiffness in the lung's interstitial tissue, which physically compresses the pulmonary blood vessels. At the same time, the immunocompromised state associated with HIV promotes smooth muscle hyperplasia within the vessel walls, narrowing the lumen from within. It's the combination of external compression and internal luminal narrowing that drives HIV-associated pulmonary hypertension.

Why is P2 loud in pulmonary hypertension?

The pulmonary valve opens against a high-pressure circuit, so when it closes, it's slammed shut by that elevated pressure — a forceful, rapid closure that produces the loud P2. It isn't the size of the valve that determines the loudness, it's the closing pressure. That's why even in conditions like tetralogy of Fallot, where pulmonary stenosis would normally produce a soft P2, elevated pulmonary arterial pressure from collateral flow can still produce a loud P2 despite the outflow obstruction.

What antihypertensives are safe in pregnancy?

After the first trimester, amlodipine and other calcium channel blockers are reasonably safe. For eclampsia or hypertension specifically during pregnancy, the preferred agent is a direct alpha agonist, such as prazosin.
