What is the management of metastatic prostate cancer?

Asked by Moderator

Metastatic prostate cancer can spread to bone or to any visceral organ, so management depends on where the metastasis is. Bilateral orchidectomy is still indicated for patients who do not want to spend money on hormone injections, and many patients choose it; it works as well as any hormone therapy injection. For bone metastasis with pain or a risk of fracture, we give bone-strengthening injections, and sometimes radiotherapy to the bone if it is weight-bearing. Once the disease becomes castration-resistant (hormone-resistant), abiraterone, enzalutamide or a similar drug is added, sometimes with docetaxel chemotherapy. Visceral metastasis is sometimes treated with chemotherapy, and newer options such as PSMA-targeted radioligand therapy and immunotherapy have come up as well. The disease spectrum is so broad that it is very difficult to summarise in one line.

What are the survival chances after surgical treatment for a patient with prostate adenocarcinoma, and is there a possibility of recurrence?

Asked by Dr. Yanik

Surgery is offered specifically in patients where the goal is cure. But every patient needs constant follow-up, with PSA testing every three to six months, because recurrence can and does happen. Nobody can promise a patient that surgery guarantees no recurrence. That possibility is exactly what the word cancer is telling us.
