In my clinic I see several couples of advancing age, with the male partner over 50. How is sperm collection and selection done, and what are the chances of obtaining good-quality sperm for implantation?
Asked by Dr. Isaya
I cannot describe the sperm selection procedure itself, because in my hospital it is a multidisciplinary process involving genetics, the IVF specialist and other doctors, and it is usually done in their laboratory. But above the age of 50 there is a high chance that sperm quality is poor and that the sperm DNA fragmentation index is increased. Unless we test for it we do not know, but even without the test there is a high chance that some sperm have poor DNA quality, which can result in recurrent pregnancy loss or implantation failure.

For patients with PCOS, how long should metformin be taken before conception and after conception?
Asked by Dr. Egbogu Stanley
Metformin can be started at least 2 to 3 months before conception where it is indicated, for example for insulin resistance or alongside ovulation induction. It is safe to take in early pregnancy, but it is not routinely continued through pregnancy for PCOS alone, because it has not been shown to prevent miscarriage; it is continued if the woman has diabetes or develops gestational diabetes. Blood sugar has to be monitored with a GTT as well as regular HbA1c and blood sugar checks.
